I hereby irrevocably authorize Katy Photography, LLC to use photographs of me, my minor child and/or my property and authorize them and their employees, assignees, licensees, legal representatives and transferees to use and publish (with or without my child's name or with a fictitious name) photographs, pictures, portraits or images herein described in any and all forms and media and in all manners including composite images or distorted representations, and the purposes of publicity, illustration, commercial art, advertising, publishing (including publishing in electronic form on CDs or internet websites), for any product or services, or other lawful uses as may be determined by the photographer. I further waive any and all rights to review or approve any use of the images or any written copy or finished product. I am of full legal age and have read and fully understand the terms of this release. I hereby release and hold harmless Katy Zimmerman Photography, LLC, its owners, employees, agents and associates from any and all claims, losses, liabilities, fines, or penalties stemming from any possession or use by Katy Zimmerman Photography, LLC, its owners, employees, agents and associates, including losses based on any expectation of privacy (reasonable or not) or confidentiality for myself and/or any minor child and/or children associated with the images specified above. Any dispute related to this agreement, or the photographs taken will be addressed in the courts of DuPage County, Illinois. Any disputes will be subject to the laws of the state of Illinois. I further acknowledge that participation is voluntary and that neither I and/or the minor child or children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in Katy Photography marketing materials or other publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release Katy Photography, LLC, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation and/or the minor children listed photographed. By selecting YES, you understand and agree to the above clause regarding the model release.
